Name and Gender Change: amend a birth certificate to recognize a court ordered name and gender change.

Gender marker change only: amend gender marker on a birth certificate to match gender identity.
As of January 1, 2019, individuals may self-select a gender identity of male (M), female (F), or nonbinary (X) to appear on their driver’s license or identification (DL/ID).
